  • Nature of Working Relationship with Partners

    In this section of the report, you will be asked to describe your working relationship with up to five EXCITE Activity 1 partners. We ask about the nature of your working relationship with partners before and after EXCITE Activity 1. A working relationship can be conceptualized along a continuum, and categories along that continuum are as follows.
  • None = None
    Networking = Establish dialogue and common understanding
    Cooperation = Match needs and coordinate efforts to avoid duplicating services
    Coordination = Share or merge resources to address common issues or to create something new
    Coalition = Share ideas, leadership, and resources
    Collaboration = Build an interdependent system to accomplish shared vision and outcomes

    These categories are adapted from the Collaboration Framework developed by the National Network for Collaboration and are not intended to imply that more intensive levels are more desirable or appropriate. We are simply interested in knowing your perception of working relationships before and after EXCITE Activity 1. 

    As you answer the following series of questions, please consider the diversity of your partnerships, including internal and external partners from various sectors, community-based organizations, healthcare organizations, and public health agencies. The CDC is particularly interested in partnerships between Cooperative Extension and local or state public health departments. If you partnered with a local and/or state public health agency during EXCITE Activity 1, please include them as one of your partners below.
